Lunch & Learn: Advance Planning

Presenter & Partner: Care Partners of Cook County
In August, we are happy to welcome our own Care Partners staff who will tell us about the Advance Planning process, including Healthcare Directives and Health Care Agents. At this session, we'll review questions to consider as we make our choices, provide a brief overview of common end-of-life medical terms, and help us know when to review or revise our advance planning documents.
A light lunch will be provided for the first 20 guests who sign up for the in-person class. If you have any food sensitivities or preferences, please bring your own brown bag lunch.
